On the quick cards

Mechanism of action

Olanzapine is an atypical antipsychotic that blocks serotonin 5-HT₂A receptors more strongly than dopamine D₂ receptors. Mesolimbic D₂ blockade treats psychosis and agitation, while 5-HT₂A blockade frees dopamine release in the striatum, so EPS are less common than with haloperidol. It also blocks histamine H₁ (sedation, weight gain), muscarinic M₁–M₅ (anticholinergic effects), α₁ (orthostasis) and 5-HT₂C receptors (appetite, glucose).

Pharmacology

Pharmacokinetics

OnsetIM within about 15–30 min
PeakIM plasma peak 15–45 min, about 5 times that of the same oral dose; oral about 6 h (label)
Half-life21–54 h (mean 30 h); longer in older adults
DistributionVd about 1000 L (about 14 L/kg)
Protein binding93%, to albumin and α₁-acid glycoprotein
MetabolismDirect glucuronidation (UGT1A4) and CYP1A2 oxidation; main metabolites inactive
ClearanceAbout 40% higher in smokers (CYP1A2 induction), 30% lower in women; fluvoxamine raises levels
EliminationAbout 55% in urine (7% unchanged), 30% in feces
BioavailabilityOral about 60% (40% first pass)
