Indications

Dosing

Bumetanide drip card and mL/h converterStart: 1 mg IV over 1–2 minutes, then 0.5 mg/h (CrCl over 75), 0.5–1 mg/h (CrCl 25–75) or 1–2 mg/h (CrCl under 25) (Brater 1998)

Mechanism of action

Bumetanide blocks NKCC2, the luminal Na⁺-K⁺-2Cl⁻ cotransporter of the thick ascending limb, which reabsorbs about a quarter of filtered sodium. Highly protein-bound, it reaches the lumen by organic anion secretion in the proximal tubule, so its effect depends on delivery into the urine. Losing the lumen-positive voltage cuts Ca²⁺ and Mg²⁺ reabsorption, the medullary gradient washes out, and extra sodium reaching the collecting duct drives K⁺ and H⁺ loss. By weight it is about 40 times as potent as furosemide; it also blocks NKCC1, including in the inner ear.

Pharmacology

Pharmacokinetics

OnsetIV within minutes; oral 30–60 min (labels)
PeakIV 15–30 min; oral 1–2 h (labels)
DurationMostly complete within 4 h at 1–2 mg; 4–6 h with higher doses (label)
Half-life1–1.5 h in adults; about 6 h in neonates (label)
Protein binding94–96% (label)
MetabolismLiver oxidation of the N-butyl side chain (label)
EliminationUrine 81%, 45% as unchanged drug; bile 2% (label)
BioavailabilityNearly complete orally, so oral and IV doses act alike (label)
Organ failureKidney failure: less drug reaches the tubule, so higher doses are needed. Liver failure: keep doses low; older adults clear it more slowly (label)
